Coordinate all clinical appointments for patients with the clinic and other medical programs including subspecialists, consultants, laboratory studies, and diagnostic testing. Contact patients with procedure date and any instructions needed prior to scheduled interventions. Follow up to assure theses interventions are completed. Contact patients that no-show for critical follow-up appointments with the clinic or specialist and reschedule. Coordinate all clinical appointments for patients with the craniofacial clinic and other medical programs including subspecialties, consultants, laboratory studies, and diagnostic testing.
Provide phone education and counseling regarding craniofacial disorders. Serve as a liaison between team members and the families. Ensuring post clinic follow through for all recommended follow-ups, treatments or referrals. Assist with contract QI projects as needed.
Work with local CMS nurse case managers on the individual needs of patients when appropriate. Ensuring coordination occurs between community agencies and families with regard to services. Assess enrollees to assure treatment recommendations are incorporated into care plans. Assure that medical information and follow up services are coordinated with CMS.
Utilize proficiency with new technology to access electronic medical records. Utilize communication skills to interact with families, and other health care providers. Liaise between clinic and school staff, CMS nurses, agency workers, mental health professionals and others to provide communication and case management needed to manage this clientele properly. Assists with the maintenance, coordination, and submitting of yearly CMS contract requirements

$25.50-$32.68 per hour
Graduate of a nationally accredited nursing program and one year of professional nursing experience. Licensed as a registered nurse in accordance with Chapter 464, Florida Statutes.
Experience with case management.
Experience with children with special healthcare needs.
Experience with Epic.
Excellent computer skills.
